In order to ensure the highest quality foot and ankle care, we will ask you to complete a brief, yet comprehensive information packet regarding your medical history and medications. Questions regarding diabetes, high blood pressure, heart conditions, arthritis, etc. are pertinent and may be relevant to how your condition should be treated with medication, or surgery if necessary. You can complete the questionnaire in our office, or, for your convenience, you may download and print the forms from our website, by clicking on the Patient Forms tab.
Oftentimes, treatment can be performed on your initial visit; sometimes, the required treatment may include a surgical procedure to correct your problem. If surgery is required, this treatment will be scheduled at your convenience. Every attempt at conservatory treatment will be performed before recommending surgical intervention. It is always our goal to deliver the most effective and least invasive solution possible in order to correct your foot or ankle problem.
